Anyone who has used SERQUAL instrument to conduct an online Survey?

I'm researching Service Quality and Customer satisfaction in the Power sector. I'm planning to use the SERVQUAL model (Questionnaire) as my research instrument, but I don't know where to get an online survey that I can send to my respondents.

Servqual has been used countless times. You can argue about the usefulness, but if you want to use it online, use google forms or something similar.
