Hello, unfortunately, I don't see the PET. From what you describe, it seems that hypometabolism implies rather dorsal than ventral brain structures, albeit not spring them. Did ADHD persist? If not, it may represent nevertheless some seizure disorder, e.g. frontal lobe epilepsy. Had she ADHD before? Would indicate preexisting condition, where attention was drawn to by the non-epileptic seizures. If not, maybe an odd variant of delirium? Attentional deficits are the core of both. Did you consider autoimmune encephalopathies?

 Dirk, hi. she was presented with stupor, which lasted for four days! We ruled out other medical condition/neurological. After several days she again returned to stupor now for three days, but we also observed hyperventilation and something like generalized tonic clonic seizures. MSCT, NMR previously, EEG all were normal as well as very details biochemistry of the blood and CSF. She has asthma only, but well controlled. Several days after she returned to previous functioning I could performed a detail psychiatric assessment and there was only ADHD. This is something present and before. She has a normal intelligence and only deficits in executive functions. I then requested PET. The findings are in line observed in people with conversion disorders and partially in ADHD. This is unusual because the two conditions do not have anything in common clinically. 

That is why I would like to see someones opinion in the PET again.

The PET findings mentioned above are too generalized. If MRI is done, then PET & MRI data can be co-registered to find out specific areas of hypo-metabolism. History is very important in any neurological case for interpreting PET brain like age of patient, presenting complaints, time since present, support system etc.. If possible any statistical neuro software can be used like NeuroQ or Neurogam for interpretation. Ideally I expect hypo-metabolism in prefrontal cortex, cingulate cortex and basal ganglion.
