Airline technology is far superior to any oher travel technology.Aircraft of today are the safest modes of transport thanks to rigorous pilot training and advanced technology.The entire duration of travel is controlled through technology.
I do not know if airline pilots are screened for alcohol or physical condition before being allowed to start the engine, but it would make sense. And since the pilot is not alone it is easier for the other staff to push the alarm bell if the pilot appears not to be fit.
A few years ago there was a discussion in Sweden of whether we should have an alco-test installed in all cars, disallowing anyone to start the engine who did not pass the test. I think the idea is very good.
The essence of the ICAO recommendation is for aircraft to report their position every fifteen minutes during normal operations. This applies only to remote areas which are not covered by surveillance by air traffic services. The IATA-coordinated Aircraft Tracking Task Force noted in its report that tracking over remote and oceanic airspace could be achieved through existing means of reporting and that new space-based technologies may play a key role in future.
