Health system worldwide has one of the most talented resourcepool, high volume of investment, and highly profitable market place. In spite of all the input, why health system cannot deliver the intended output?
It does reasonably well. I think our expectations keep getting higher, which is Good!
Health systems may fail to deliver intended outputs and outcomes for a number of reasons including but limited to:
• A lack of excellence across healthcare facilities within the health system
• Unsafe and ineffective use of medicines, devices, and other technologies
• Ineffective use of health information systems
• Short-term financing mechanisms that don't support continuous quality improvement.

A healthcare system with a weak leadership, unsupervised and unsystematic practical training for newly graduated healthcare providers, unsafe medical practice, low patient satisfaction and uncontrolled distribution and prescription of medications, create an atmosphere for fatal medical error and medical waste. Therefore, growing awareness in improving population’s health is significantly important to the growth of a society for developing countries where the healthcare system is immature, brittle, and susceptible. The fragile areas of health care system that is significantly important and in need of immediate reform include:
• Lack of administrative and leadership expertise
• Medication administration without regard for standard guideline
• Misuse and overuse of prescription antibiotics
• Low patient satisfaction with the quality healthcare provided by the public facilities
• Government leadership

Many experts have presented meticulously their views & suggestions. Why health system doesn't match the pace of increasing demand?
1. Health is considered only a lip service by many till it deteriorated.
2. There's dichotomy of health care delivery (i) the focus more on curative than preventive (ii) there's social disparity as emphasis was more in urban than rural remote & slums. Now these areas have been covered under National Health Mission.
3. The critical points (a) lack of political commitment (b)less allocation of budget on health care services.
4. The major issues to be addressed like :
* Client satisfaction
* Quality services
* Accountability
* Demand from community
The main principle of Primary Health Care ( Universal Health Care) not judiciously in operation like community participation and it's still the government is operating but community not fully participate.
People should participate & support in planning, organising, implementation, observing/supervision , budgeting & evaluation at every level.
5. Shortage of skilled human resource & lack of motivation/ commitment among health personnel.
6. Political interference in posting & reluctance on part of health care providers to serve in rural remote difficult areas.
7. Lack of basic amenities in difficult areas & little check & balance for irregularities.
8. There's need of total professionalism, periodical reorientation/ training & incentives for good health care providers .
9. Create Missionary spirit and dedication among health personnel.
10. Strengthen coordination among various stakeholders.
