There is this missconception that ARV adherence is suboptimal in developing settings, but in my perspective there are some logistical issues that must be addressed before we drawn such conclusions. (1) lack of optimal 1st line regimens less toxic , conditions that could mining good adherence , (2) Long distance to get HIV clinics , (3) lack of sufficient drug stockpile and least, but not least , the fact that viral load monitoring is not the standard of care for HIV monitoring

Thank you very much José for your answer. In my country I used to see that many patients prefer long distance to get HIV clinics. Because they avoid some friends and the neighbor. Last week, I received a patient who has given up her ARV therapy since six months. Her neighbor works at the hospital. She avoids him.
