Where to obtain the scoring & administering procedure for Job Crafting Questionnaire (JCQ)?

Hi all,

Does anyone know where I could get hold of the scoring & administering procedure for the Job Crafting Questionnaire (JCQ)? The questionnaire is free to use but no explicit instructions were provided as to how JCQ should be administered. Would really appreciate if anyone could share some information on this. Thanks!
