I'm trying to conduct research regarding the initial lean waste assessment in a production line and I saw some of the papers using the waste assessment questionnaire as a tool. Those papers mention that it was based on the 68 questions and I couldn't find any source for those questions. So my question is, are there pre-determined questions for the WAQ or should we create it as we like from the data gathered from the production line?

As different organization has different systems, you have to customize the questionnaire accordingly. At first try to identify your scope and objective. It will be then easier to modify the questions. Of course literature review and experts opinion is also helpful.
