A 40-year-old man, who the last month is complaining of pronounced fatigue, tiredness.

Past illnesses: 2 years ago diagnosed with subacute thyroiditis, with hyperthyroidism (TSH-0,009, T4-37,1), was treated by endocrinologist; after 2 months the tests were normal. His family history is unrevealing.

His vital signs are within the normal range and the clinical examination is normal.

White blood cell count is normal (5,65x[10.sup.9]/L) but with 17.0% eosinophils (0,88x[10.sup.9]/L), neutrofiles=39%; normal percentage of other blood cells; hemoglobin is 160 g/L, thrombocytes – at normal level - 226 x[10.sup.9]/L; ESR=2.

Liver, renal, thyroid gland function tests are normal.

Tests for viral hepatitis , HIV– negative. 

Tests for parasites in the stool and serum are negative ( Taenia solium, Toxoplasma, Toxocara, Ascaris lumbricoides, Strongyloides, Echinicoccus, Trinchinella spiralis, Giardia Liamblia, enterobius vermicularis).

The test for total IgE is slight elevated (=113; norma till 100).

The work-up for rheumatologic diseases were also negative (CRP, Reumatic factor; anti-nuclear, anti-neutrophilic cytoplasmic antibodies).

Was consulted by hematologist who excluded hematological malignancies ( but bone marrow aspiration was not performed ).

The abdominal + thyroid gland ultrasound + Echocardiography - without changes.

Chest X ray -without pathological changes.
