This is a huge subject and has been explored in detail for many cancer types. Carcinogens cause cancer by several mechanisms. The most common is DNA damage leading to mutations in genes such that tumor suppressors are inactivated or oncogenes become activated. Another mechanisms is repeated tissue injury and inflammation by chronic environmental stimuli, which can lead to cancer as the inflammatory environment provides cytokines and growth factors to drive proliferation. Anyhow, there are entire books on this subject.
it is a very as interesting as complicated question! It is very hard give an answer, because of it depend from the sort of cancer, there are many causes (from the genetic to the environment). I only study the heavy metal pollution in environment, so I can say you that food, water and air can be the principal sources of metals for our bodies. Under the ecological point of view, the main problem is the "biomagnification" because it is cause of a great accumulation of metals (as other kinds of pollutants) in the food chain. For instance, the fact of eat big fishes is a higher risk factor than eat little fishes considering the consumption of mercury. Mercury can cause disease which involve the neurological system, liver and kidney. As well as cadmium (for lungs). Considering the different kind of metals there are different kind of target organ, the same is for other kind of organic and inorganic pollutants such as dioxin and furans, IPA etc, this is the reason why is so difficult give an answer, because a cancer may be caused by several kinds of pollutants or a pollutant can be the cause of several kind of cancers.

as we al know that cancer was caused by carcinogenic material, i think you need to be more specific for your question or research about cancer.
as i know many factor who caused this disease (cancer), it could be caused by genetic herretible disorder, or caused by carcinogenic goods for example heavy metal in water, air or soil pollution.
