As we know high altitude pulmonary edema (HAPE) is a noncardiogenic pulmonary edema which happens in lowlanders who ascend rapidly to altitudes greater than 2500-3000 m. What do you think is its pathophysiology ?
1. Exaggerated hypoxic pulmonary vasoconstriction and pulmonary hypertension
2. Increased capillary pressure and pulmonary capillary hypertension
3. High permeability due to ‘stress failure’ of the pulmonary capillaries
4. Reduced fluid clearance from the alveolar space
