At the National Cardiothoracic Centre in Accra 60% of our open heart cases are congenital/paediatric with some degree of Pulmonary hypertension. We routinely use preoperative sildenifil for them and intraoperative milrinone. We will start ECMO soon. However current literature has mentioned nebulised prostacycline and milrinone. Any experiences with these will be appreciated.
