Inhaled prostacyclin are expensive also and the benefit for the patient (especially in the perioperative time) is uncertain.

Hypocapnia, normoxia and alkalosis(but not by usage of NaBicarbonate, as far as the patient is not on ECMO) are the best pulmonary vasodilators But before pleural effusions have to be evacuated.
