Kane & Trochim (2007) have a book, Concept Mapping for Planning and Evaluation, that gives considerable detail about their general approach.

Personally, I think their approach is unnecessarily elaborate (and I have some technical doubts about how they accomplish their merger of multidimensional scaling and cluster analysis).
