4 years old boy with unilateral Rt ear microtia and preserved bone conduction ABR waves down to 30 dBHL
The other ear in anatomically and radiologically normal BUT with absent ABR waves for both click and 500 Hz tone burst stimuli.
What is the best line of treatment for this younger kid?
What iam asking is to use bone conduction HAs oe BAHA for microtia ear OR CI of the profound ear
I agreed with previous comments. Reconstruction of the pinna can be planned when the child is 7 and above
OAE is absent.
It's not ANSD.
Its straightforward case of profound SNHL for the none atretic ear
BC aid on the microtic ear, possible BC implant in the future (I prefer Bonebridge types rather than BAHA).
Further immediate evaluation of the left, because if the loss is profound he is running out of time for a CI to benefit the left optimally
BC aid/ BAHA/ soft band BAHA to rt ear immediately. it will help language development a lot. we dont know the AC ABR level in that ear. in any case BC aid would be the first step i would try out.
with rehabilitation and progress in therapy, CI for other ear can be thought of. CI would be more for better binaural hearing- speech perception in noise etc.
How ever considering CI would depend on many other factors- cost, is there support for CI nearby, cost of spares etc.
Hopefully the child is doing well now.. as the post is almost 9 months old.
