I have a question, I will conduct a community nutrition intervention in 4 schools to improve diabetes management for children (by changing dietary behavior, raising awareness and exercising) (I measure glucose levels during the 12-month study period and determine if there is improvement before and after the intervention) Is the correct study a cohort study?

Allow me to participate in this question if you promote an intervention in some of the groups, the study design would no longer be an observational cohort and would become a prospective intervention study, the most appropriate term would be that of a community trial before and after the intervention.
