I will simply suggest to go to website of an organization (Higher Education Commission, Pakistan; Harward university, Oxford univ etc), go to their quality control site, download forms and modify according to your own desire, requirement and suitability. As such such evaluation questionnaire perhaps will not be available to meet your needs.

One of the areas I found in my research about teacher evaluation forms is that they rarely measure cultural competence (ability to teach students from diverse cultural backgrounds), so that the evaluation data do not speak to issues of cultural insensitivity in the classroom. However, are almost meaningless when used for summative purpose for comparing outcomes between individual courses or assessing the performance of individual staff.

1. axis of the curriculum (course)

Is evaluated how they were organizing the course and whether the material presented was good and   whether that course profile was clear and gave adequate information on the course and the level of involvement of students

2. Teaching axis (Ledger)

Here are Is that the spoke of teacher  was clear and coordinator and if he respect the students and if he  Interested on subject of course  Did he give enough time to student out of class.. ect.
