If the patient is given empirical therapy given at the hospital setting and later on culture is found to be non sensitive or rather viral etiology. What should be done? Should we stop the antimicrobial(AMA) then and there and start the AMA to which the pathogen is sensitive or de-escalate the existing regimen and start the new sensitive regimen ?
Yes, because the original regimen is useless and you've got applicable solutions from culture or sensitivity results then why stick to the former agent.
Some prefer to complete the course of the AMA to prevent the resistance.
Have seen some clinicians to deescalate the AMA and start with the other one simultaneously.
Even I think it should stopped then and there if you find nonsusceptible even if you are in the half duration of the regimen.

First of all continuing treatment caused microflora gone and the way for opportunist micro organism is really easy.
In other hand some antibiotics release to nature and cause resistance issues.
