Pilot studies can often add a lot of information to systematic reviews in areas where not much research has been conducted. Yet including them could be seen as unrealistic as those interventions being tested still preliminary which means the data could bias the conclusions.

Interesting question. I think the answer depends on the research question of the review. If it is a meta-analysis, there might be methodological problems with including pilots. However, if the aim of the review is to describe interventions and discuss content and interpretation of clinical practice, I think that pilots are adequate to include.
