A lot of research is showing that learning is not so technically media-dependent at all, so that student learn more effective by reading on paper versus on screen as example. RCT studies on learning are very difficult to do, and there are extremely few of them.
On the other hand, access to teaching for learners is technically media-dependent.
You are asking about a questionnaire, but I would suggest you look also into the huge amount of quantitative information that most online platform can give you. From time spent on a page, video, task till number of times logged in answers given (correct), amount of words written in an an assignment etc. The ultimate test is how many participants complete the course.
