I think this value represents a multidrug resistance (more than 3 antibiotics).
Well some research have done the foundation and the limit of 0.2 have been established just any other standard established by EUCAST or CLSI fo antimicrobial zone of inhibition or breakpoint. However, I will suggest you do not focus so much on MAR because its application limited to the current information on antimicrobial being used within a geographical region. Therefore comparability is limited, instead I always prefer multi-drug resistance based on WHO definition of resistance to at least one antimicrobial in at least three classes of antimicrobials. This is comparable to other region and other study.
