Yes, it is possible but very rare. Definitely, there will be some revisions suggested by the reviewer, it may be minor revisions or it may be major revisions. One has to welcome the reviewers' suggestions, it is an opportunity to learn from the expert people of that particular subject domains.
I think acceptance or rejection of paper depends upion the quality of research itself and the perspective view of reviewer... Sometimes expert of relevant research is not reviewing ur paper... Then it might be minor rejection..
But when we see the queries which we think are irrelevant... We can send our paper to another journal... There is no enforecement..
