Although there are some anecdotal postive mentions in literature, there is little evidence for its effectiveness. There is only one RCT done on this drug in combination with topical steroids (Agarwal et al in 2005, see file) and they did not find a stronger response as compared to topical steroids combined with placebo. We do not use it as a treatment option in our pigment disorder clinic, so I don't have personal experience with this agent.
Although it gives valuable information, personal experience can not be compared with a randomized controlled trial. On the other hand only 1 RCT is performed on a relatively small group of patients. If many clinicians notice postive effects of levamisole, it is maybe time for a new trial.
To my knowledge narrow-band is presently considered a treatment of choice for vitiligo . Unfortunately , acral areas ( mainly fingers and toes ) are unresponsive to this approach ; I would like to know if levamisole was employed in this clinical form and its effectiveness.
