Like lack of building structures of hospitals where it matters, equipments in hospital, efficient management hospital processes, qualified doctors, good pay for nurses, food pay, shift hours for nurses etc can be problems that affects short in achieving general positive outcomes (@ James Shanahan)
The provision of holistic care would begin with bridging the gap between patient and healthcare provider needs. The nurses themselves might be facing concerns or lack of support/resources that they require and evidence based literature on this can help understand these.
Additionally, finding simple ways for nurses to elicit from patients, what they need and then the system around them being developed in ways where the nurses can take action on this (greater resources from the hospital) etc.
Training usually centers around directives rather than identifying nurses needs and addressing them. I believe this can be helpful.
