If you need people to evaluate and give input to your survey form (instrument), you should provide PDF or doc file so that people can see. The way you have it now is people would have to fill our (complete) the survey as respondents----that is not the way to seek advise. If someone is competent in methodology and instrumentation, one look at your blank survey is enough to tell you whether your "survey form" passes the muster.
