I am looking for a database that contains lists of COVID-19 patients along with their symptoms and details; for instance: male/female, age, fever temperature, oxygen saturation level, presence of headache, loss of appetite, runny nose, and physical condition (critical or non-critical). Dataset related to COVID-19 prognosis analysis would be helpful.

I don't think you will get all of this information in any database. You can get them in hospital folders.
