I had developed three surveys, for my MEd degree (Educational Technology) concerning students', teachers' and administration's perspectives on implementing elearning practices at the University of Guyana (my home university). The three published papers, published between Aug-Oct 2015, are on my RG webpage. However, I do not have the surveys as appendices in the published articles.

I agree with some of the colleagues mentioned above suggesting a self-developed questionnaire, and my experience of doing so was quite rewarding as well. My tips are to 1) engage teachers, and curriculum leaders and graduate student research assistants, collect their feedback on the questionnaire and go through a few rounds of revisions, 2) make sure the questionnaire design is contextualized and meeting the needs of pedagogical practice and student current practice of technology use, and 3) the survey items are well structured and enable the researchers to calculate the reliability of the survey items.

Hi..I've used similar questionnaires for purposes like yours. Please review my research papers on my RG page, especially this one on "authentic language learning" (Making it 'authentic'....)..Moreover, you'll find lots and lots of studies that employed questionnaires in EFL writing on Google Scholar..Just use keywords like those ones for searching: EFL students' perceptions of using ICT - how EFL learners view ICT - ICT in EFL writing - English learners' attitudes towards ICT - Using ICT from EFL teachers' perspectives - etc. With my best wishes..Mahmoud
