Do you know a reliable way to ask the computer to compare the photos?

Si por su experiencia clínica en la valoración de mejoría de una lesión desde el punto de vista de imagen de las "arañas vasculares" en un antes y en un después de cierto tratamiento, son básicas para la comparación las imágenes obtenidas por medio de fotografías o videos de las lesiones.
Si el material a comparar es básicamente fotografías, es conveniente obtenerlas por medio de fotografías de alta definición, de preferencia fotos digitales, para una ulterior comparación por medio de un software específico de micro-computación.
Para esta valoración de imágenes en un antes y después se pueden utilizar herramientas de programación de inteligencia artificial, como por ejemplo en programación C ++
En la actualidad existen herramientas muy avanzadas para procesar y comparar imágenes, una de ellas se denomina MATLAB, que permite visualizar y evaluar por comparación objetos (imágenes) en 2 y 3 dimensiones.
Al comparar las imágenes, sugiero utilizar la siguiente escala:
Igual imagen (IA) antes y después (ID); IA = ID; por tanto no hay mejoría pero tampoco empeoramiento.
Imagen Antes (IA) vs Imagen Diferente (#) Después (ID); IA # ID; por tanto, puede haber mejoría o puede haber empeoramiento, dependiendo del tamaño de la lesión.
Mejoría: Imagen Después más pequeña que al principio: IA > ID
Empeoramiento: Imagen Después más grande que al principio: IA < ID
Éxito = Total Desaparición de la Imagen (Araña Vascular)
A esta codificación para comparar y evaluar mejoría, le puede aparear la escala porcentual propuesta por usted
