I'm looking for a questionnaire or survey the focuses on "reason for migration". I know there are questionnaires that include "reason for migration" items, but they tend to be focused on acculturation or some form of trauma.

Dear Patrick Jackson, I don't have an example of questionnaire but you can find the outcomes of focal groups combined with national statistics responses in this report on migration in Bolivia (starting from page 57). Depending of the context, age, gender and others you can find different reason sucha as work migration, education opportunities in Bolivia, but I think that in other places there are reasons like armed conflict or environment destruction. Hope this can be useful.
It is simply because migrants like the country/culture, and this is the justification for the integration where both migrants and host community reconcile themselves to living together. That is called "acculturation."

I had not thought of It and since I am exploring this situation having never experienced it, I am not aware of the possible pitfalls of participation. I recognize now, I need to ensure I show compassion and a sense of empathy in my questions about this topic. Thank you again.
In response to your question, while trying to solve this problem, I learned there is no instrument specifically designed to address this question, it is a point acknowledged by the US Census Bureau and the United Nations, two agencies, I would expect would be most interested in examining this phenomena. So, my intentions are to ask a couple questions in the demographic section of the questionnaire.
One way of offering a level of protection is by making the questionnaire anonymous. The respondents' origin is not as relevant to my study as the fact that they relocated to a new country they did not relocate and that those who migrated have been in the new location at least 12 months. Additionally, my concern are what push/pull factors influenced their decision to leave or not leave their place of origin. While the possibilities of factors are significant, I will limit the list to common factors listed in several studies on migration. I will categorize factors such as dissidence and the like to simply politics, violence and trafficking to safety to make the responses more palliate.
