In response to viral infection via respiratory system a mucus defense associated with the primary inflammatory response 1 to 3 days after infection, to be followed by immune reactivity some two weeks later.So, is the primary response the most serious problem in this CORO19 infection, producing mucous plugging or is the ground glass matter indicative o0f direct pathology>
There seems to be different responses in different individuals. There is the viral pneumonia picture where the respiratory mucosa is stripped and then there appears to be a cytokine response in some individuals. There is one theory that the higher the initial viral load the worse the response. However, I have not seen this published anywhere but would account for why some young doctors are not surviving given the procedures performed. Additionally, the ARDS chest xray seems to not corrolate to clinical symptoms, some patients have had this discovered incidentally on a shoulder xray and others having saturations in the 80s but being comfortable without oxygen. There seems to be a timeline where patients have a incubation time of 2-5 days (up to 14) then at around day 7 they develop dyspneoa. Day 9/10 they become septic if they are going to and within two days develop ARDS. The depending on severity and location either die or get discharged between day 18-22. Alot of this is new territory and I am not convinced that there is enough literature to definatively say exactly what happens.
