The best way to screen for diabetes type 2 is screening of high risk groups not all the community. It can improve the cost-effectiveness of the program. High risk groups for diabetes type 2 are people more than 30 years old, people who have diabetic person(s) in their family, people of high BMI, women who had pregnancy diabetes, women whom their infant's birth weight was more than 4 Kg and ...
The best way is to screen all people of more than 30 years old and search for other risk factors in them. If they have one or more we should test their blood sugar with glucometer or FBS test.
There is no other way to study the prevalence of Type 2 Diabetes Mellitus in society , except by screening . The major problem is that DM , Hypertension , Obesity & Lipid disorders are asymptomatic at onset & these risk factors can be picked up only by screening . Starting treatment early would be of great benefit to the individuals in an era of rapid increase in life style diseases . I agree with Ali Jafari about the indications for screening for detection of risk factors . I feel that health care is a very important matter for any country & economic factors should not deny effective screening for non communicable diseases .
Community based screening to diagnose Non Communicable Diseases ( NCD ) is important & is not expensive . Measurement of height , weight , BMI & abdominal circumference needs a simple HT - Wt machine & measuring tape , BP can be measured by a BP apparatus & blood sugar needs a glucometer .If the person on screening has Obesity , Hypertension or Diabetes Mellitus , they can be referred for further investigations . The screening can be done by doctors or trained nurses or paramedical staff . A detailed history about family history of Diabetes Mellitus , smoking , physical activty & dietary habits can be obtained with a questionnaire . It is essential to train the staff to do the survey effectively . The only expensive part is the strips to be used in the glucometer . But this is nothing compared to the treatment of Non communicable diseases . Early diagnosis of risk factors & treatment of Diabetes , Hypertension , Lipid disorders & Obesity is the only solution to reduce the burden of NCD .
