Particularly with reference to the NHS of UK, it can be said that the general physicians are required to be actively involved in patient care, especially in the case of hospital discharge and rehabilitation after trauma. But to what extent the pharmacist can be involved too? What powers, authorities, and skills can the pharmacist apply to relieve an ailing patient?
An increasing involvement of pharmacies in the territorial health is an essential step in the creation of an innovative Health System. The pharmacies can give to the citizens the opportunity to:
• a fair and uniform access to the services offered by the National Health Service,
• find the latest generation of drugs near home, in order to facilitate the admission of patients to innovative therapies,
• have access to preventive screening, because the pharmacies can increase the adhesion of citizenship to these life-saving protocols,
• integration with new processes of taking charge of the citizens for monitoring the evolution of chronic diseases and acting as office information on services.

Currently they only review repeat prescriptions and give gps advice on medicines optimisation, could this role be extended? The profession of - clinical pharmacist - has evolved and established in the last two decades. Clinical pharmacists advice to the medical and nursing staff in matters of proper use of drugs ( drug- drug interaction, adverse drug reaction, polypharmacy pharmacogenetics , biological drugs, dosimetry and any other pharmacological issue) Their contribution is essential and there even are publications describing how clinical pharmacists participate and contribute to the medical rounds. Community pharmacists do about the same in ambulatory systems. They do not advice to the patients but to health care staff .

I'm writing a protocol study to assess the feasibility for the pharmacist to deliver to patients suffering from low Back Pain a new tool for coordination of primary care. Study will end on February 2017.

Pharmacists have long had an important,yet often unrecognized role in primary and secondary health care. There has been studies showing that pharmacists as part of the primary health care team has led to improved glycaemic control in primary care patients. Many pharmacists,unlike doctors, have not published studies which underly their roles in patient care and hopefully this can be changed.
Pharmacists also provide information to patients about drug use, major adverse reactions, drug interactions. However there role in compliance with medications especially in patients with polypharmacy cannot be over emphasized.
How do we get pharmacists involved? We simply ask them to be involved in quality projects and at the meeting of the heads of various departments requesting what involvement needs to be done.
