I have never done this before. So far I have collected scales for different factors after detailed research.
I plan on taking items from them and maybe restructuring them to make a proper questionnaire.

You include items assessing integrity. Many scales have them, For instance, Quintax, The authors also provide information on scale development.
Generally, look at papers that have developed measures - they will provide a useful background.
