I made a psf file by using only Chain A, 1 NAD and 1 FMT from my pdb that originally contains 4 chains, 4NAD and 8 FMT.
1) NAD is getting split up when making psf file.
2) FMT originally was inside chain and now its in the corner.
I even tried by using all 4 chains but it still gives me same error. Can anyone suggest how to solve it please.

PDB file is a plain-text file: go back to the original file with four chains: remove all lines that do not have "A" in the 22nd column.

Hi,
Did you extract this pdb and psf from DCD trajectory?
Incase you did, before extracting psf and pdb from DCD run "wrap" command.
