The relationship between the dose and the plasma concentration/ or effect of a drug is completely different form adult to neonates and pediatrics. Most of the doses used today in pediatrics and neonates should be checked regularly for accuracy and to obtain maximum effect and reduce the side effects. For this purpus TDM is very essential for every drug especially those which have narrow therapeutic index...Phenytoin, Carbamazepine......

BUT, the problem now how can we obtain the most recent (updated TDM for each drug) and it should be accurate (is there is any official source in 2018 we can depend on it for TDM in pediatric and Neonates)
