I have a question related to severity assessment of ADR and that is if a patient is hospitalized due to some ADR caused by drug and has reported 6 adverse drug reactions for example, neutropenia, pancreatitis, bladder cancer, headache, rash, cough, then how would I be able to tag the ADR or classsify the ADR on the basis of Hartwig and Seigel scale or how would I be able to find that which individual ADR caused hospitalization as there are multiple ADR in practicing?
I have to put the ADR into Level 1, Level 2 and so on. (hartwig and seigel scale)
I am not sure I understand your question well. So I'll try to answer you.
I would use two scales to do that.
For the causality "Naranjo’s causality algorithm" or or the "modified Karl-lasagna scale". Classify de drug as cause of the ADR as possible, probable, definitive, each adverse effect.
For the severity of the adverse effect, H-S Scale. A neutropenia can be moderate, medium or severe, for example.
A drug that has ever registered the adverse effect of neutropenia, will be an unlikely cause, but the severity of the AE can be severe. --> That tells you that there may be another more likely cause than that drug.
Regarding what caused hospital admission, that is not measured on a scale. You will see that in the hospital admission order. Although the drug has adverse effects of neutropenia, pancreatitis, headache or rash, the patient will be admitted to the hospital for one or the other.

I suppose that this is exactly the case when you should address a question to a physician (better to those, who made a decision on hospitalization),
Most likely there was only one real reason for hospitalization. It is possible that experienced doctor will consult you right and only 1 occurence will be realy severe and could be classified as the reason for hospitalization.
