Is this possible to ask an editor not to send a manuscript to a former colleague due to the personal reasons? If this is possible, what is the most effective ways to exclude a reviewer whom you don't trust to make a fair and standard review?
Many journals which use Editorial Manager or ScholarOne for uploading manuscript typically has an option to include opposed reviewers. If not, you can mention the list of opposed reviewers in the cover letter. Please refer the attachment.
Note: Only few journals use double blind review process. Hence, the author names are mostly known to the reviewers.

Many thanks for answering my question. Please consider that the journal that I want to submit my manuscript has a single blind review: authors are known, reviewers are anonymous.
