Bilateral knee replacements can be done at any age. People who may face some complications are those who have kidney problems (there is potential risk of kidney failure) and heart dysfunction or failure due to valve stenosis (can lead to pulmonary oedema) or atherosclerosis (with the risk of CVA) due to the increased pressure from the tourniquet. In the case of bilateral arthroplasty I prefer not to use tourniquet and the patient goes for the first one or two days in HDU or ICU according to Hospital's facilities to control the input and output of the fluids.
Thank you. We need to take their pre op ASA grade also in account. We do not take pts higher than grade II. Moreover, these patients and family members must be quite motivated and understand the risks and benefits of the proc edure .
ASA grading, consent and cooperation by a motivated patient are the most important points in simultaneous bilateral TKA. Anyway, it is a demanding procedure that increases perioperative risks.
