A report from a paper? if it means a summary or an outline of an assigned article, then it would be the way you summarize an article, which has its traditional form as explained on the net (look at wikihow for example). Also your instructor should determine what you are expected to zoom on in in your report. In general, you need to read and comprehend the stuff and then say it in your own words but in a shorter and (if possible) in a more organized form.

I agree with you about your words.
That question was for the time I didn't start working on my project. Fortunately now I know what should I do and I have passed some stages of it.
