Not sure what exactly you mean but the L5-S1 disc is degenerated.
You can find information on MRI classification of disc degeneration in Pfirrmann's paper published in 2001.

1- do histogram equalization
2- do global shareholding binarization or Otsu and dilation for the black
3- use bucket fill algorithm to group all the segmented areas.
3- the vertebrae and disc are connected and they will form a large group
4- remove the smallest groups from the image leaving the largest group(s).
as shown in the image the green and pink are the largest groups this include the spine area.
you may design a special erosion filter that remove all pixels only if they are connected and surrounded by let us say 10x10 connected pixels
