Wellen's syndrome has been described as deep inverted T waves on precordial leads V1 to V3 and V4, associated with severe proximal left descending coronary artery obstruction (>80%). This syndrome was originally described in patients with unstable angina and T-wave findings were found to be prognostic markers of anterior myocardial infarction (75% to 85% positive predictive value).

Nice example of Wellens syndrome in NEJM, but it's very well known and I'm rather surprised it was published in such a high impact journal. I've seen many such cases over the years, although admittedly the pictures are nice! In a sense, Wellens syndrome is only detectable from the ECG, as it is defined by the ECG. If there is not the anteroseptal T wave inversion, then it isn't Wellens syndrome - at least that's my understanding. By the way, anyone thought about the cause of the T wave changes - ie the electrical causation - clearly it's been an episode of ischaemia/infarction, but why are the T waves inverted for a few weeks?
There are two types of Wellens syndrome
1. Deep T wave inversion anterior chest leads
2. Biphasic T waves in anterior chest leads
