Most UK Universities would exempt you from English Language tests if you have done your previous studies in English 

Personally, I would say that if you struggle with the TOEFL test, you would also struggle with writing an English-language thesis.

I understand that your English language should be strong to deal with papers and research but also  I can not get those skills with out English environment, the time also treats a lot of problems, the institute or the research unit must be sure about the qualifications of  her enrolled , but in the time of research not before that , suppose I have all required skills of this research topic but with a low score in IELTS can improve it in the research time, Do not you think this system is one of the worst ways to lose science

If you were educated in English medium school or university, you are not required TOEFL or IELTS tests.  Or, you may try universities in France and Germany

According to the  Cambridge English Language Assessment site, both the Proficiency exam (CPE) and the Advanced (CAE) are recognized by universities, as a means of accrediting the starting level of course applicants from abroad: http://www.cambridgeenglish.org/news/view/worlds-top-universities-use-cambridge-english-exams/

If a student has one of these, (s) he presumably will not need to do the IELTS. If (s) he does not, but is familar with the task types from ,say, the FCE (s)he may find it easier to do the CAE than the IELTS. ( I would not claim that the Proficiency is easier than any other well-known exam anywhere!). Another reason for opting for the CAE is that the IELTS hasn't taken off everywhere. In some countries it may be difficult to find IELTS group classes, or even one-to-one  teachers with ample experience teaching the IELTS.

Another question is which exam is the best preparation for university study. I would say the IELTS academic test is the best one that I know, as the reading and writing parts are specifically designed to fit the way these skills are applied at university. However, there are even questions over the suitability of the IELTS -see e.g. "IELTS Washback in Context: Preparation for Academic Writing in Higher Education". 

The university of Science, Malaysia (USM) which is one of the best and reputable public universities would help you in that.
