I do not think that there is a validated questionaire of the kind that you are looking for. The best approach would be for you to work with others who are recognised for their work in this area and to create a questionaire. You need to include negative answer and questions to assess respondents approach to the questionaire. This can take the form of a 5 point Likert scale. This can be tested on a group of about 20 students and their answers validated statistically (a Z-test). Once you have a statistical value, then you can do your survey.
