It is not possible to review your questionnaire without answering the questions. Can you publish the plain questionnaire in say .pdf form so we can look at it?
Second, you have to establish internal and external validities for the questonnaire.
Third you have to compute the Cronback Alpha for it as a double check.
Fourth, you have to check and measure the test- retest reliability for the questionnaire.
THESE ARE NOT SIMPLE TASKS if your questionnaire is to withstand critical analysis and be used to provide useful information.
