If a journal and conference belong to the same entity(like EAI or ACM), and the journal has specifically asked you to give them an extended version of the conference paper, do we need to be concerned about self plagiarism?? What all aspects we should be aware of when we extend a paper like that??

In general, no. In these cases, I would add a footnote in the journal paper stating something such as "An earlier version of this paper was presented at the Annual Conference of ...."
