The poorer outcomes following solitary pancreas transplantation, as opposed to simultaneous pancreas kidney transplantation, are often attributed to a relative inability to monitor for immune mediated graft injury. Furthermore, it has been argued that bladder drainage is advantageous, specifically in this group of patients, as it facilitates monitoring for and therefore early detection of such injury. I am not sure these arguments are defendable and would be interested in views.
