the most used is the Separation Anxiety Test (SAT) , for children from 4 years old.

There is also the Attachment Story Completion Task (Bretherton et al. 1990) for children from 3 years old.

For adults the most famous is the Adult Attachment Interview (AAI)

there's also the Child Attachment Interview, developed by the Anna Freud centre in london, for older children and teens.  What age are you interested in?
