Please Let me know where you have worked on vertical integration of services - i.e. not making differentiation in services for infrastructure or applications. I am looking for a way to provide a single SLA / contract for management of applications, without the client having to manage performance separately for applications and for environments managed by vendor.

for me it's not very clear what do you mean by the client managing the performance of different levels of the application (e.g. the app, the DB, SO and the VM). If it's only the specification of the SLA, we developed a SLA definition language for clouds, which supports the specification of multiple services in the same SLA. For example, you can define that the VM has 2 GB of ram and, at the same time, the availability of the application (99.3%).
