Most Diabetes surveys are focused on urban population considering it is primarily an urban problem because of changes in life style, diet habits etc. But studies have also shown that it is equally prevalent among rural population and many of them didn't know that they were sufferers. Since Diabetes is a silent killer (because of asymptomatic nature of many sufferers in early phase), early detection & initiation of treatment is needed by the community to limit the target organ damage caused by it.

My question is what could be a suitable community-based health care model to reduce the burden of uncontrolled & undiagnosed diabetes in rural India where there is hardly any diagnostic & treatment facilities for Diabetes?
