Could anyone provide me with the scoring of the Almost Perfect Scale- Revised questionnaire?

I haven't been able to clearly find any article which explains on how to score the APS-R by (Slaney et al., 1996). It is a 23-item questionnaire and has 3 sub scales in it. Please do provide me with a scoring for that questionnaire. Thanks!
