If any one did include any consent questions (few lines) in a tenant satisfaction survey. For example, we use de identified data from different housing providers tenants for benchmarking purposes. Any example of consent question so tenants know that their de identified data is used for benchmarking.? any help will be appreciated.

YES, you should add in a consent statement at the top of your questionnaire/survey. I have one from my end:
Consent Statement
Thank you for participating in our survey. We would like to inform you that your responses will be de-identified and remain anonymous. The information you provide will be used solely to further the objectives of this study. By continuing with this survey, you acknowledge that you have read and understood this statement, and you consent to the use of your de-identified responses for the purposes of this research.

Feel free to modify it to better fit the specific needs and context of your study.
Note that you do NOT need to tell any participant that you're pilot-testing your questionnaire items since this issue is NOT an ethical one, but a methodological one. Letting participants know that the items are not yet validated will lead to some not caring about doing the survey and giving you half-hearted, dishonest responses. So instead, say that their participation will advance the objectives of your study, as highlighted above.
