You might ask on the American Chemical Society site. https://communities.acs.org/t5/Ask-An-ACS-Chemist/bd-p/AskACSChemist They have an entire Division dedicated to Safety as well as a long-standing training handbook for both academia and industry. Someone there might know if there are prepared surveys available.

A follow-up to this discussion. There are several good studies about the usefulness of this approach. The attached study includes the questionnaire, and thus may be what you are looking for. The second one may be more useful in assessing educator's understanding of lab safety readiness. Other forms to audit such procedures and usage are also available.
